Provider Referrals

Information for physicians, pediatricians, schools, and fellow therapists referring a child for occupational therapy.

How to refer

Send a referral by fax or phone and we'll take it from there. If it's easier, have the family call us directly and mention your name. We'll follow up with you for the paperwork.

The simplest route for a referral form, chart notes, or an evaluation report. Include a contact number for the family and we'll reach out to them directly.

Please don't send protected health information over unsecured email. Fax it, or call us and we'll arrange another secure way to receive records.

Helpful to include

  • Patient name, date of birth, and caregiver contact information
  • Referring provider name, NPI, and preferred contact method
  • Diagnosis and relevant ICD-10 codes
  • Reason for referral and specific areas of concern
  • Relevant medical history, recent evaluations, or reports
  • Insurance information, if you have it

What happens after you refer

  1. We contact the family

    We reach out to the caregiver directly to answer questions, walk them through the process, and get intake paperwork started.

  2. Comprehensive evaluation

    The child's first appointment is an occupational therapy evaluation: history review, caregiver concerns and priorities, observation, and appropriate standardized assessment.

  3. You receive a written evaluation

    The family receives a written evaluation summarizing findings and recommendations, and we share it with referring providers where appropriate.

  4. Ongoing communication

    Progress is monitored throughout the plan of care. We communicate with providers when appropriate and complete re-evaluations as required by insurance and clinical guidelines.

We work alongside you, not around you

Therapy extends beyond the clinic walls. We collaborate with physicians, teachers, lactation consultants, and other providers so the same strategies follow a child across every environment, and so nobody is working from a different plan.

Schools and daycares: if a child on your campus would benefit from coordination with our team, call us. We're glad to talk through goals and carryover strategies.
